A Clinical Analysis of Abdominal Trauma

Abstract
This report is a retrospective clinical study of 116 patients who were admitted to surgical department of Dae Rim Saint Mary's Hospital with abdominal trauma during the period from Mar, 1988 to Feb, 1991.
@ES We obtained following results:
@EN 1) Among the 116 cases of abdominal trauma, explolaparotomy was performed in 94 cases(81.0%).
2) The sex distribution was 89 males(76.7%), and 27 females(23.3%) with ratio of 3.3:1 and the age distribution revealed a high incidence in third decade(36.2%) and fourth decade(31%).
3) Among the 116 cases of abdominal trauma, blunt injuries were 89 cases(69.8%), and penetrating injuries were 35 cases(30.2%). According to the cause of injuries, traffic accidents(32.8%), fall down(8.6%) and gun shot wound(0.9%).
4) Among the 94 cases that explolaparotomy was performed, 63 cases (67.1%) of injured patients were operated within 8 hours after injury.
5) The clinical symptoms and signs were abdominal pain(93.1%), abdominal tenderness(86.2%), rebound tenderness(66.4%), abdominal rigidity(61.2%) and decreased peristasis(47.4%).
6) The plain films of abdomen showed free air under the diaphragm in 18 cases(38.7%) among 49 cases of hollow viscus rupture and abdominal paracentesis was performed in 54 cases among 81 cases of blunt abdominal trauma, of which 33 cases(61.1%)
revealed positive findings and its accuracy was 87%.
7) The abdominal ultrasonography was performed in 32 cases among 81 cases of blunt abdominal trauma, its accuracy was 84.4% and the abdominal C.T scanning as performed in 14 cases among 81 cases of abdominal trauma, its accuracy was 92.9%.
8) The involved intraabdominal organs, in order of frequency, were small bowel(19.0%), mesentery and omentum(18.5%), spleen(10.7%), liver(9.5%), colon(6.0%) and pancreas(5.4%). Ileum was the most common site of small bowel injuries.
9) The methods of operative measurements were variable.
10) Blood transfusion was needed in 61 cases(64.9%) of 94 patients Massive transfusion above 6 pints was required in 8 cases(8.5%).
11) According to admission duration, 10~20 days was most common and the average admission duration was 21.9 days.
12) Postoperative complications were developed in 32 cases(34%), which were pulmonary complication(13 cases), wound infection(9 cases), intestinal obstruction(7 cases) in order of frequency.
13) The overall mortality rate was 5.2%, who had abdominal trauma with multiple injuries and the main causes of death were respiratory failure, sepsis, hypovolemic shock.
